Whites Creek Wellness and Rehabilitation Center, located in Davidson County, was owned by Whites Creek Holdco, LLC as of the first quarter of 2026, the Centers for Medicare and Medicaid Services (CMS) reported. Whites Creek Holdco, LLC has held ownership since June 1, 2019.

The facility operates on a for-profit basis.

During this period, the nursing home earned an overall four-star CMS rating, surpassing Tennessee’s average of 3.0 for nursing homes statewide. CMS assigns ratings based on essential factors, including health inspection results, staffing metrics, and resident care quality.

Records show Whites Creek Wellness and Rehabilitation Center averaged 107.2 residents per day in the first quarter of 2026, offering 127 available beds. The facility does not operate with an affiliated hospital.

Data from the first quarter of 2026 indicate the facility had no fines or penalties assessed against it.

Davidson County, Tennessee, was home to 16 for-profit nursing homes in total.

Estimates from the U.S. Census Bureau show the nation’s older adult population is rising while younger populations decline. The number of Americans 65 and older climbed by 13% from 2020 to 2024, whereas the population under 18 decreased 1.7%. The older population has seen dramatic growth over the past century, increasing from 4.9 million in 1920 to more than 61 million in 2024.

Research published in 2021 indicates that almost half this group—about 28 million people—may need long-term services and support, with projections suggesting that over a third may eventually reside in nursing homes.

“Living a long life is something that many of us want and could get,” said Jesse Slome, executive director of the American Association for Long-Term Care Insurance. “But when we live a long life, the chances of us needing long-term care increase exponentially. But when you need that type of care, there are limited options.”
